In keeping with the COVID-19 guidelines, we request you assist us when attending a clinical appointment.

• BRING YOUR OWN PERSONAL PROTECTIVE DEVICES. For any appointment to be effective, you must be comfortable with the protection you have .

• To avoid overcrowding in the waiting room, we request you WAIT UNTIL 5 MINUTES PRIOR TO YOUR SCHEDULED APPOINTMENT BEFORE ENTERING AND CHECKING IN.

• Practice social distancing and limit the number of people in the waiting room by ATTENDING YOUR APPOINTMENT ALONE, IF POSSIBLE.
